Scripts for Administering Tests

☐ 9 Point Motor Function

  1. This test is going to measure your eye alignment.
  2. You will see a small red dot on the screen.
  3. Follow the dot with your eyes as it moves around the screen.
  4. Try not to move your head, but you may blink if you need to.

☐ Near Point of Convergence

  1. This is going to test your eyes’ ability to work together at near.
  2. Do you see the line and dot?
  3. I'm going to move this card toward you. Say TWO when you see the one line turn into two lines.
  4. Now I'm going to move the card away. Say ONE when you see only one line again.

☐ Cardinal Gaze Position

  1. This test will measure how quickly each eye moves in different directions.
  2. You will see a solar system in the center of the screen.
  3. An arrow will shoot out from the solar system with an alien at the end.
  4. Blow up each alien by looking at the red cross on their face as quickly as you can, then look back at the solar system.

☐ Four Dot Fusion

  1. This test will gauge how well you’re seeing out of each eye.
  2. . On the next screen you will see a few dots.
  3. Tell me what you see.

☐ Circular Smooth Pursuit

  1. You will see a white dot on a black screen.
  2. The dot will move in a circle.
  3.  Follow the dot with your eyes only. Do not move your head.

☐ Horizontal Smooth Pursuit

  1. You will see a white dot on a black screen.
  2. The dot will move back and forth to the left and right sides of the screen.
  3. Follow the dot with your eyes only. Do not move your head.

☐ Vertical Smooth Pursuit

  1. You will see a white dot on a black screen.
  2. The dot will move up and down to the top and bottom of the screen.
  3. Follow the dot with your eyes only. Do not move your head.

☐ Horizontal Saccades

(Admin notes: you can toggle the green dot on/off on the test screen. By default, the dot will turn green when the test-taker looks at it, so that you know they are going back and forth between dots.)

  1. You will see 2 white dots near the left and right sides of the screen.
  2. There will be a 3-2-1 countdown in the middle of the screen. Look here first.
  3. When the 1 disappears from the countdown, look back and forth from one dot to the other as quickly and accurately as you can. Do not move your head.
  4. The test will not start unless you are looking at the countdown. Do not look at the dots until the 1 disappears.
  5. (If green dots turned on): Ignore the green dots. That’s only for me to know you are looking at the dots. If it is distracting, we can redo the test and suppress the green dots.

☐ Vertical Saccades

(Admin notes: you can toggle the green dot on/off on the test screen. By default, the dot will turn green when the test-taker looks at, so that you know they are going back and forth between dots.)

  1. You will see 2 white dots near the top and bottom of the screen.
  2. There will be a 3-2-1 countdown in the middle of the screen. Look here first.
  3. When the 1 disappears from the countdown, look back and forth from one dot to the other as quickly and accurately as you can. Do not move your head.
  4. The test will not start unless you are looking at the countdown. Do not look at the dots until the 1 disappears.
  5. (If green dots turned on): Ignore the green dots. That’s only for me to know you are looking at the dots. If it is distracting, we can redo the test and suppress the green dots.

☐ Simple Reaction Time

  1. Place your finger on the "1" button.
  2. You will see a solar system in the middle of the screen.
  3. Whenever the solar system changes to an alien, press “1” as quickly as you can.
  4. The test will only continue if you’re looking at the solar system.

☐ Choice Reaction Time

  1. Place your fingers on the "1", "2", and "3" buttons.
  2. You will see a solar system in the middle of the screen.
  3. An arrow will point out from the solar system in different directions.
  4. At the end of the arrow will be an image.
  5. Press “1” if you see an alien, “2” for an astronaut, or “3” for the planet Earth.
  6. After each button press, look back at the solar system for the next arrow to appear.
  7. The test will not continue until you look back at the solar system.

☐ Discriminate Reaction Time

  1. Place your finger on the "1” button.
  2. You will see a solar system in the middle of the screen.
  3. An arrow will point out from the solar system in different directions.
  4. At the end of the arrow will be an image.
  5. Press “1” if you see an alien. If you see an astronaut or planet Earth, do not press anything.
  6. After each button press (or decision not to press), look back at the solar system for the next arrow to appear.
  7. The test will not continue until you look back at the solar system.

☐ Fixation Stability

  1. For the first stimuli, make sure to read the instructions before pressing enter to begin.
  2. You will see a series of 6 shapes on the screen for 7 seconds each.
  3. When the shape appears, stare (or fixate) on the center point of the screen. Once fixated, press enter to begin.
  4. The shape will disappear after 7 seconds, and you can relax your eyes.
  5. A new shape will appear. Stare (or fixate) on the center of the screen and press enter to start.
  6. Repeat Steps 4-5 for the remaining shapes.

☐ Contrast Sensitivity

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder.)

  1. This is a two-part test.
  2. Place your hand on the directional arrow keys (up, down, right, left).
  3. In the first section, you will see two circles.
  4. If you see the stripes on the right circle, press the right arrow key.
  5. If you see the stripes on the left circle, press the left arrow key.
  6. In the second section, you will see one circle, with either horizontal (left-right) or vertical (up-down) stripes.
  7. If the stripes are horizontal, press the right arrow key (6 on the number pad)
  8. If the stripes are vertical, press the up arrow key (8 on the number pad).

☐ Static Visual Acuity

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder.)

  1. Place your hand on the directional arrow keys (up, down, right, left).
  2. You will see the letter C in different sizes.
    1. If the C is opening to the right (normal C) press the right key.
    2. If the C is opening to the left, press the left key.
    3. If the C is opening upward (U-shape), press the up key.
    4. If the C is opening downward (horseshoe), press the down key.
  3. If you can’t see it, don’t guess – it extends the test unnecessarily.

Dynamic Visual Acuity (DVA) 1: Head Moving, Object Still

(Admin note: Be sure the speaker volume is turned on)

  1. Place your hand on the directional arrow keys (up, down, right, left).
  2. Look at the black box on the screen. You will hear speakers emit a 2hz metronome sound.
  3. Turn your head slightly to the left or right in sync with the metronome while looking at the black box.
  4. When you see a letter C appear in the black box, press the arrow key that matches the direction the C is facing/opening.
    • Opening to the right/normal C, press the right arrow key
    • Opening to the left, press the left arrow key.
    • Opening upward (U shape), press the up arrow key.
    • Opening downward (horseshoe), press the down arrow key.

Dynamic Visual Acuity (DVA) 2: Head Still, Object Moving

  1. Do not move your head during this test.
  2. Place your hand on the directional arrow keys (up, down, right, left).
  3. The letter C will shoot across the screen from left to right. Press the arrow that matches the direction the C is facing/opening as quickly and accurately as you can.
    • Opening to the right/normal C, press the right arrow key
    • Opening to the left, press the left arrow key.
    • Opening upward (U shape), press the up arrow key.
    • Opening downward (horseshoe), press the down arrow key.
  4. Your speed and accuracy are important on this test.

Dynamic Visual Acuity (DVA) 3: Head Moving, Object Moving

  1. Your head is free to move at all times.
  2. Place your hand on the directional arrow keys (up, down, right, left).
  3. The letter C will shoot across the screen from left to right. Press the arrow key that matches the direction the C is facing/opening as quickly and accurately as you can. • Opening to the right/normal C, press the right arrow key

☐ RightEye Reading EyeQ

(Admin note: You will select the grade level at the beginning of the test. After the comprehension questions, you have the option of testing again at the same grade level or selecting a different grade level. To continue to the next RightEye test in the protocol instead, select “Skip Test”)

  1. When you are ready to begin click, “Start Reading Test”.
  2. Read the story to yourself – do not move your head, do not read out loud.
  3. Press the enter key as soon as you finish.
  4. On the next page, you will answer 10 comprehension questions.
  5. When you are finished answering the questions, click Submit.

Training Games: Maze Master

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder. You have the option of selecting level of difficulty 1-3 and Active Gaze at the beginning of the test. Active Gaze displays where the eyes are looking in real time during the test.)

  1. You will see a maze with numbered balls throughout in numerical order.
  2. Complete the maze by looking at the numbered balls in order as quickly as possible.

Training Games: Cosmos Combat

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder. You can also select the difficulty level 1-3 and the increase the distance of matching images from the center)

  1. There will be a 3-2-1 countdown in the middle of the screen. Look here first.
  2. You will see an image (alien, astronaut or planet Earth) in the center of the screen surrounded by four other images.
  3. Look at the center image, then look at the matching image from the surrounding images.
  4. For example, if there is an alien in the center, you will look at the matching alien that is surrounding the center alien.
  5. Your speed and accuracy are important for this game.
  6. Return to the center after each match.
  7. Repeat until the game ends.

Training Games: Bubble Blast

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder. You can choose a starting direction (N, S, E, W, NE, SE, NW, SW), difficulty level 1-3, and size of the bubble (100%, 75%, 50%).

  1. There will be a 3-2-1 countdown in the middle of the screen. Look here first.
  2. Pop the bubbles with your eyes only as the bubbles move up.
  3. Follow the bubble with your eyes only as it goes back down. The bubble will turn green when you are on target.

Training Games: Space Storm

(Admin note: You have the option to test both eyes together, or the left or right eye individually using the provided occluder. You can choose number of ships to defend (1-2), level of difficulty 1-6 and toggle distractors. Space Storm can last up to 60 seconds and can be repeated up to 10 times. You have the option to change number of ships, difficulty level and number of ships before each game. If you do not want to repeat the game, select “Skip Test”)

  1. You will see a spaceship in the middle of the screen.
  2. There will be a 3-2-1 countdown above the ship. Look here first.
  3. When the 1 disappears, aliens and asteroids will appear and move toward the spaceship.
  4. Shoot the asteroids with your eyes only to protect the spaceship.
  5. If an alien or asteroid reaches the ship, the game is over
  6. (If distractors are turned on) If you see a red alien, do not look at it. Red aliens will not damage the ship and you will lose points if you look at them.
