When administering Sensorimotor, start with the Screener and when medically necessary proceed to the Exam. Use the optional symptom questionnaire at any time. Once the assessments are complete, document your findings and next steps in the Interpretations & Recommendations report.


Sensorimotor Screener

Test Purpose  Instruments Administration Method Script

9 Point Motor Function

This test measures ocular alignment in multiple fields of gaze by measuring each eye’s location as they look at a red dot placed in all 9 cardinal gaze positions.

None

Adjust the VisionTracker2 to align the patient’s eyes in the middle of the left eye-box. They should be between 55-60cm from the screen.

You will see a small red dot within a white circle. Follow the dot with your eyes as it moves around the screen. Try not to move your head, but you may blink if you need to."

Near Point of Convergence

This test is used to determine the ability of the patient’s eyes to converge on an object at near.

Accommodation Convergence Ruler and Target Cards

1. Place the target card on top of the ruler and position it at the 53cm mark.

2. You should be directly in front of the patient and in a lower position.

3. Place the tip of the ruler on the bridge of their nose at a 45 degree angle downward from their eyes.

4. Move the card toward the patient at a constant speed of 1-2cm per second. Instruct the patient to look at the vertical line and say TWO when they see two lines instead of one. This distance is the Break Point. You should also observe their eyes as you move the card closer. They may not notice double vision, but if you see one or both eyes stop fixating on the card, this distance is the Break Point.

5. Record the Break Point then move the card away from the patient at 1-2cm per second. Instruct the patient to say ONE when the two lines become one line again. The distance at which they see only one line is the Recovery Point. Record it.

"Do you see the line and dot? As I move this card toward you say TWO when you see the one line turn into two lines. Now I’m going to move the card away. Say ONE when you see only one line again."

OPTIONAL: Convergence Insufficiency Symptom Survey (CISS)

The Convergence Insufficiency Symptom Survey (CISS) is a questionnaire used as both an assessment tool and an outcome measure in treatment of convergence insufficiency.

Show patients the CISS questionnaire while seated at the VisionTracker2 and ask patients to complete the survey.

"Please answer the following questions about how your eyes feel when reading or doing close work."

Next Steps After the Screener

Based on the results of the 9 Point Motor Function test, Near Point of Convergence test, and/or the optional Convergence Insufficiency Symptom Survey, the results screen will display either "Medical necessity determined" or "No medical necessity determined." Click "Register" to create a User ID for the patient and continue to the Sensorimotor Exam, or click "Finish" to exit.

Cardinal Gaze Position

Cardinal Gaze Position is a sensory test of binocular function that records the speed of each eye when moving in different directions.

Occluder

Adjust the VisionTracker2 to align the patient’s eyes in the middle of the left eye-box as shown in the right eye box. They should be between 55-60cm from the screen.

Using the occluder, cover each eye when prompted by the on-screen instructions.

"You will see a solar system in the center of the screen. An arrow will shoot out from the solar system with an alien at the end. Blow up each alien by looking at the red cross on their face as quickly as you can, then look back at the solar system.”

Four Dot Fusion

This test is a sensory test of binocular function using red-green glasses.

Red-Green Glasses

1. Place the red-green glasses over the patient’s eyes while they're seated about 33cm away from the screen before starting the test.

2. Once the patient has verbally confirmed that they see the dot(s), continue to the next screen (don't have them remove the glasses before proceeding to the next screen).

3. On the next screen, click the box that best shows what the patient described. You may also have them select the appropriate box.

4. Enter any clinical notes in the bottom before continuing.

“On the next screen you will see a few dots. Tell me what you see.”

Next Steps After the Exam

After completing the Screener, Exam and/or the optional Convergence Insufficiency Symptom Survey, you're ready to complete the Interpretation and Recommendations report. Here you can record your findings, select conditions, and recommend next steps. This can be done directly from the VisionTracker2 system or accessed online via your RightEye Dashboard.
